We understand that every patient’s needs are unique, and we’ve established our office policies to make your visits as convenient and efficient as possible. Please take a moment to review our guidelines concerning appointments, new patient forms, dental emergencies, and rescheduling below.

Appointments

New Patients

If you are a new patient, please arrive 20 minutes before your scheduled appointment time. You may download all the necessary new patient forms from our website, complete them ahead of time, and bring them with you to your first appointment.

Returning Patients

If you are a returning patient, we request that you arrive at least 10 minutes before your scheduled appointment. This extra time helps us check you in and update any information as needed.

New Patient Forms

New patients can download, print, and fill out the forms on the New Patient Forms page. You can either mail the forms to us or bring them with you to your first appointment.

Dental Emergencies

If you experience a dental emergency, please call our office at any time, even after regular hours. If the office is closed, a recording will provide instructions on what to do and who to contact.

Rescheduling & Cancellations

At our office, we value every patient and make meticulous arrangements to accommodate their individual needs and availability. If you find it necessary to reschedule, we kindly request that you contact us at least 48 hours prior to your appointment. We encourage you to refrain from last-minute cancellations whenever possible.
